Advanced Clinical Practitioner (ACP) – Home Visiting Service

At IC24, we’re looking for an Advanced Clinical Practitioner (ACP) to join our East Sussex Home Visiting Service. This is an opportunity to work at the forefront of community-based care, delivering high-quality clinical assessments and treatment to patients in their own homes while supporting wider urgent and primary care services.
As an experienced clinician, you'll work autonomously within your professional scope of practice, providing assessment, diagnosis, treatment and prescribing for patients presenting with acute, urgent and undifferentiated conditions. You'll play a key role in helping patients receive the right care in the most appropriate setting, reducing unnecessary hospital admissions and improving patient outcomes.
What you'll focus on
→ Delivering advanced clinical care in the community
You'll assess, diagnose and manage patients in their own homes, providing high-quality clinical care for a wide range of urgent and primary care presentations.
→ Making safe, evidence-based decisions
You'll use advanced clinical reasoning skills to determine appropriate treatment plans, referrals, investigations and follow-up care while managing clinical risk effectively.
→ Independent prescribing
As an Independent Prescriber, you'll prescribe medication following clinical assessment and examination, ensuring safe and effective patient-centred care.
→ Supporting continuity of care
You'll work closely with GPs, nurses, allied health professionals, NHS 111, community services and secondary care partners to ensure patients receive coordinated and seamless care.
→ Contributing to service improvement
You'll support audits, clinical governance activities, shared learning and quality improvement initiatives that enhance patient outcomes and service delivery.
→ Developing clinical excellence
You'll maintain your continuing professional development, contribute to mentoring and shared learning, and help promote best practice across the wider organisation.
This role forms part of our East Sussex Home Visiting Service, supporting patients across the community. You'll work as part of a highly skilled multidisciplinary team delivering responsive care where it is needed most, helping patients avoid unnecessary hospital attendance wherever appropriate.

• Enjoys working autonomously while remaining part of a wider multidisciplinary team
• Is confident assessing and managing urgent and undifferentiated presentations
• Has strong clinical reasoning and risk management skills
• Communicates effectively with patients, carers and healthcare professionals
• Enjoys delivering care within patients' homes and community settings
To be successful in this role, you'll need:
• Professional registration with the NMC, HCPC or GPhC
• Independent Prescriber qualification
• Advanced assessment and minor illness qualification (Level 6 or 7)
• Proven experience of autonomous clinical practice
• Strong clinical decision-making skills and ability to manage complex presentations
• Excellent communication and interpersonal skills
• Commitment to continuous professional development and reflective practice
• Evidence of service improvement and innovation

Hours
2 Positions available at 37.5 hours per week
