As Care Coordination Manager, you'll be the key operational leader ensuring our Primary & Community Care services run efficiently, safely, and seamlessly. From rota planning and staff management to overseeing pharmacy operations and fleet logistics, you’ll be instrumental in maintaining high-quality, patient-centered care.
This is a high-impact role where you'll collaborate across clinical, non-clinical, and operational teams—solving real-time service issues, analysing performance data, and leading quality improvements that benefit patients and staff alike.
What You’ll Be Doing:
- Lead service planning, coordination, and daily operations for IC24’s P&CC division
- Manage and develop the Rota, Administration, and Pharmacy teams
- Monitor performance data to identify trends and proactively improve service delivery
- Act as the main escalation point for operational challenges and planning queries
- Oversee vehicle logistics, medication distribution, and compliance processes
- Prepare reports and updates for internal stakeholders and commissioners
- Contribute to incident investigations, learning, and service improvement plans
What We’re Looking For:
- Proven experience managing in a primary care, health, or social care environment
- Strong leadership skills with the ability to motivate, support, and develop teams
- Excellent communication, problem-solving, and analytical skills
- Solid understanding of rota systems, quality improvement, and service metrics
- Flexibility to support out-of-hours service delivery when required
- A passion for delivering outstanding care and continuous improvement

Hours Available 37.5 hours per week, Flexible
